基礎研究

半月板再生

　以前は外傷による半月板断裂や加齢に伴う変性断裂に対しては、関節鏡を用いた部分切除～亜全摘術が行われていました。しかし、切除した半月板は元の形状に戻らず、長期間の経過で関節軟骨の損傷や、関節が変形する変形性関節症が生じる可能性高いことが問題となっています。そのため、近年では半月板機能の重要性が再確認されており、断裂半月板は可能な限り縫合し、温存する考えが広がってきています。しかし、断裂の形状によっては縫合が不可能な場合や、欠損が大きい場合はやむを得ず部分切除になるケースが少なくありません。また、半月板縫合後の再断裂など問題点が多いのが現状です。
　そこで当科では、ウサギの膝半月板を用いて欠損した半月板を再生する研究をしています。複数系統の細胞に分化できる能力（多分化能）と多分化能を維持できる能力（自己複製能）を持ち、なおかつ周囲組織を刺激し自己治癒能力を促進する効果も期待できる脂肪由来幹細胞を用いた半月板再生の研究をしています。

1. 脂肪由来幹細胞シートを用いた半月板治療の検討
　ウサギの膝半月板に欠損部をつくり、欠損部に脂肪由来幹細胞をシート状に作成した三次元構造体を移植して半月板の再生促進を試みています。まだ、動物実験のレベルですが、脂肪由来幹細胞の移植で半月板の再生が促進されることが証明され、国内外の学会で発表しています。

2. 脂肪由来幹細胞シートを用いて再生された半月板様組織の力学的特性の評価
　ウサギの再生された半月板に関しては、正常な半月板とは見た目は異なっているものの、半月板と同様な機能を有していることが期待できます。そのため、今後の臨床応用に向けて、再生された半月板に対する力学試験を行なっています。

腱・靭帯損傷治癒過程促進

　膝前十字靭帯損傷に代表される関節内の靭帯損傷は自然治癒することが少なく、靭帯縫合術の成績も不良であるため、靭帯再建術が行われています。靭帯再建術は、自身のハムストリング腱や膝蓋腱を用いるため、手術が成功したとしても、選手が受傷前の状態に戻るのは難しいといわれています。
　そこで当科では、靭帯や腱がもつ再生能力に注目し、靭帯再建術に替わる治療方法の開発を目指して東海大学工学部、福井医療大学と共同研究を行っています。

1. アキレス腱由来＂gel＂の再生能力の評価
　マウス、ウサギのアキレス損傷部の断端より分泌された＂gel＂を回収し、その＂gel＂に張力をかけることで新たな腱へと再生する性質を発見しました。より再生能力の高い＂gel＂の性質について研究を重ねています。

2. アキレス腱由来＂gel＂の治癒促進能力の評価
　再生能力の高い＂gel＂を靭帯損傷部に添加し、正常の靭帯損傷の治癒過程と比較することで＂gel＂がもつ治癒促進効果の評価を行っています。

スポーツによる全身骨格筋活動のポジトロン断層撮影法（PET）を用いた評価

　臨床において、がんの局所診断や脳血流量の計測、心筋虚血・再灌流の評価などに利用されているPETは、運動後の骨格筋活動の評価にも有用であることが分かってきました。この評価方法では一般的に行われている筋電図による評価とは異なり、より運動環境にそくした動作の評価が可能であり、同時に表層から深部まで幅広い評価を行うことが可能となります。
　われわれはスポーツ損傷予防トレーニングに力を入れており、世界中で最も普及している予防プログラムであるFIFA11＋の骨格筋への効果とバランス能力の向上を評価し、より日本人の体格・動作に合わせたトレーニングとして最良と考えられるオリジナルのスポーツ損傷予防トレーニングプログラム「Ready for play-金大整形トレ-」を制作し、完全無料アプリとして発表しました。
　今後はそれぞれの競技による特有の骨格筋活動を検討し、その特性に合わせたトレーニングプログラムを構築していくことで、各スポーツに特化したパフォーマンスの向上とスポーツ損傷予防へと繋げていくことを目的とし、随時アップデート予定としています。

高位脛骨骨切り術

　大腿骨頭の中心から足関節に引いた線を、「荷重線」といいます。日本人の変形性膝関節症では、この荷重線が膝の内側を通るO脚（がに股）が多く、歩く度に膝の内側に負荷がかかります。高位脛骨骨切り術では、荷重線が中央から外側になるように矯正する手術です。
　この手術法では、人工膝関節置換術と違い、自分の膝を温存できるので、手術後にも正座やスポーツ復帰することが可能です。

（手術適応）

　荷重線が内側を通る変形性膝関節症で、外側の軟骨や半月板が正常であること、膝の可動域（屈伸可能な範囲）が良好であること、注射や運動療法などの保存的治療を十分行なっても効果が乏しい方、そして人工膝関節をするには年齢が若い方やスポーツ・趣味などで活動性が高い方が適応になります。
　喫煙者は、骨切りした部位の骨癒合が遅れることが多く、少なくとも術前3ヶ月は禁煙する必要があります。

（手術治療）

　まず、膝の関節鏡を使って傷んだ半月板の修復を行います。その後、脛骨の内側から外側に向かって骨に切れ目をいれて、くさび型に広げた状態でチタン合金のプレートとネジで固定します。プレートとネジが体内に入っていてもMRI検査は問題なく行えます。
　骨切りして開いた部位は自身の骨で埋まってきますので、手術後1.5年で支えていたプレートとネジを抜去します。治療終了後には特別な行動制限はありません。
