基礎研究・臨床研究の紹介

臨床では人体で最も精密な器官である「手」の疾患を中心に扱っています。手の創傷や脱臼・骨折、ばね指などの腱鞘炎、手根管症候群、肘部管症候群など手指のしびれを生じる絞扼性神経障害、手指の変形性関節症、リウマチに伴う手の変形や腱断裂など、手外科班が対象とする疾患は多岐に渡りますが、各々の疾患に対して最善の治療が行えるように、日々研鑽しています。また、手外科班では欠損した軟部組織や骨などを再建する組織再建術にも積極的に取り組んでおり、拡大鏡や手術用顕微鏡を積極的に活用し、骨だけではなく、皮膚や脂肪、筋肉、腱、神経、血管などの軟部組織に関してもきめ細やかに配慮した治療を心がけています。医学は日進月歩の世界ですが、学内ではカンファレンスや抄読会を行い、学外では国内外の学術集会や勉強会に参加することで、常に最新の知識を持ち治療に望める体制を整えています。
また、臨床における問題点を解決するための基礎研究を随時行っています。現在は脂肪幹細胞を用いた神経再生の研究や、神経の癒着予防に関する共同研究などを行っています。また、医工連携の基礎研究として母指CM関節のバイオメカニクスに関する研究を行っています。 「手」は小さな器官ですが、まだまだ検討すべき問題が残っているのは間違いありません。先人の業績にあぐらをかくことなく、常に最善の治療とは何かを追求していきたいと考えています。

脂肪由来幹細胞による末梢神経再生促進効果について

脂肪由来幹細胞は各種の成長因子を分泌するとされており、末梢神経再建の分野でも神経再生を促進する可能性があることが判明している。神経の再建には自家神経、人工神経、同種処理神経が移植神経として実際に用いられている。当科ではそれぞれの移植神経に脂肪由来幹細胞を様々な方法で投与する研究を行っており、その治療成績について検討している。

ゲル製剤を用いた神経の癒着防止に関する研究

外傷や手術操作により「癒着」が生じ、癒着により神経が圧迫、絞扼され神経の血流障害や牽引損傷を生じたり、手術における剥離操作が困難となる例が存在する。腹部外科領域では腹腔内の癒着防止剤としてフィルム状の製剤が臨床応用されているが、神経に対しては臨床的ニーズの高さにも関わらず、未だに臨床応用できる製剤が存在しない。神経周囲に使用する際には神経の走行に沿って広範囲に広がりその場に留まる性状であることが重要と考え、ゲル状の製剤について神経癒着モデル、腱癒着モデルを用いた基礎研究を進めている。

母指CM関節のバイオメカニクスに関する研究

母指のつけ根の関節である「母指CM関節」は、鞍関節という特殊な形状を有しており負荷が加わりやすく、変形性関節症が問題となっている。母指CM関節症の変形性関節症は中年期以降に罹患率が高い疾患であることから、今後さらに患者さんが増えることが予想される。私たちは母指CM関節の運動様式や関節面の適合性の評価や、変形性関節症に至る過程の評価を行い、母指CM関節症に対する新しい治療方法を開発することを目標に金沢大学工学部との共同研究を継続している。

腓腹神経採取後の下肢の愁訴に関する調査

末梢神経が外傷や腫瘍などにより欠損した場合、健常な神経を採取して移植する「自家神経移植術」が最善の治療方法と考えられているが、神経採取部に生じるしびれや痛みが問題視されている。しかし、健常な神経の採取に伴う愁訴の程度や経過については詳細な報告が少ない。そこで、当科では下肢の知覚神経である「腓腹神経」を採取し自家神経移植術を行った例における下肢の愁訴に関して調査を進めている。

橈骨遠位端骨折の疫学調査

手くびの骨である「橈骨」は、転倒した際に骨折することが多い部位である。北陸地方では冬季に気温が氷点下となり路面が滑りやすくなるため、特に冬季に転倒による「橈骨」の骨折が多発している。当科ではどのような気象条件の際に骨折例が多くなるのかを評価している。また、「橈骨」の骨折は骨粗鬆症の初期に起こることが言われており、全国的にも啓蒙活動が進んでいる。当科でも「橈骨」を骨折した方にきちんと骨粗鬆症治療が行われているかを調査して、次の骨折を起こす前に骨粗鬆症の治療に取り組めるように努めている。これら調査は金沢市内の主要な病院と協力して行っている。

手根管症候群の術後経過に関する調査

末梢神経障害の障害部位を診断する電気生理学的検査について

ヘバーデン結節に対するすず製固定装具の観察研究（UMIN試験ID:UMIN000026637）

DIP関節の変形性関節症に対する、すず合金製の指輪型装具を開発した。その成績について報告する。対象は有痛性のDIP関節の変形性関節症の30例であり、内訳は女性28例、男性2例、年齢は46～85（平均68.0）歳であった。装具はすずに微量の銀を配合した合金製であり、骨棘部が装具と接触しない指輪型のデザインとした。装着頻度や期間については指示せず疼痛に応じて自由に装具を使用するように伝えた。装具装着前および装着後6か月までの期間にDIP関節の可動域、疼痛に関するVAS、Hand 20による評価を行うと共に、疼痛が軽減した時期の問診や、装具の装着の容易さ、整容面に関する10段階の満足度評価（0が不満、10が満足）などを行った。疼痛に関するVASは装着前58.4から装着後1か月時31.1へと有意な改善を認めた。Hand 20は装着前35.0から装着後6か月時20.2へと有意な改善を認めた。DIP関節の可動域には有意な変化を認めなかった。疼痛が軽減した時期は装着後10日以内とした例が最多だった。装着の容易さに対する満足度は8.9、整容面に対する満足度は7.6となっていた。本研究の結果、すず合金製装具は装着の容易さや整容面に対する満足度が高くなっており、装着後早期に疼痛を緩和する効果があると考えられた。すず合金製装具はDIP関節の変形性関節症に対する保存療法の選択肢の一つとなる。

We made a tin ring splint for osteoarthritis of the distal interphalangeal joint which looks attractive and is easy to wear. We report the treatment results with this splint. We enrolled 30 patients with painful osteoarthritis of the distal interphalangeal joint in this study. A tin ring splint was made with tin alloy containing small quantities of silver. Patients were instructed to wear the splint when they felt pain. Patients were assessed before splint use and after 1, 3, and 6 months of splint use. Endpoints included the numeric pain scale, active arc of motion of the distal interphalangeal joint, Hand 20, functional assessment criteria of the upper extremities, and treatment satisfaction. Additionally, data were collected on time to symptom relief and satisfaction related to usability and appearance of the splint (0 = dissatisfied, 10 = satisfied). The numeric pain scale showed significant pain improvement from 58.4 at baseline to 33.1 at one month, and Hand 20 score also showed significant improvement from 35.0 at baseline to 20.2 after 6 months. Active arc of motion were not changed significantly. Most patients responded that symptoms were relieved by the 10th day after treatment. Satisfaction related to usability was 8.9, and appearance was 7.6. A tin ring splint quickly reduced pain, and satisfaction related to usability and appearance was high. This splint could be one choice for conservative treatment of osteoarthritis of the distal interphalangeal joint.
