基礎研究

前十字靭帯再建術における移植腱の組織学的研究

　前十字靭帯（ACL）再建術に使用する移植腱として、半腱様筋腱が多く用いられますが、小児において半腱様筋腱（ST）を用いた再建術は再手術率の高さが問題となっています。一方、近年欧米を中心に大腿四頭筋腱（QT）を用いた良好な術後成績の報告が散見されています。しかし、移植腱自体を対象とした研究は少なく、骨が未熟な例においても十分な初期強度を有するのかという点に関しては明らかになっていません。そこで当科では年齢層別にST、QTの組織学的評価を行い、骨が未熟な小児でも大腿四頭筋腱が移植腱として優れていることを明らかにすることで、小児ACL再建術の成績向上を期待しております。

1. 半腱様筋腱（ST）・大腿四頭筋腱（QT）の構造の電子顕微鏡学的評価
　未熟群、若年群、成人群の各年齢群における平均コラーゲン線維径は、ST・QTともに、成長とともに増加しました。この結果をST・QTの両者で比較すると、成人群および若年群の平均コラーゲン線維径にはほとんど差がないことがわかりました。ただし、未熟群ではQTの方が大きい結果でした。コラーゲン線維径は移植腱の強度には相関関係があり、再断裂や術後の不安定性の要因である可能性があります。この結果は、特に骨格的に未熟な小児において、QT が ACL 再建にとって優れた移植腱である可能性があることを示唆しています。

2. 半腱様筋腱（ST）・大腿四頭筋腱（QT）・膝蓋骨腱（PT）におけるコラーゲンタイプの割合の研究
　半腱様筋腱（ST）、大腿四頭筋腱（QT）、膝蓋骨腱（PT）におけるコラーゲンタイプの割合を調べる研究を行っております。術中の余剰組織から得られた腱組織に対し、I 型および III 型コラーゲンについて免疫染色を行い、視覚的および定量的に評価した結果、若年時のST には、強度的にⅠ型より弱いⅢ型コラーゲンがPT・QT よりも高い割合で含まれていることが示されました（PTは100% I コラーゲン、QT は 1% の III 型コラーゲン、STは34%のIII型コラーゲンを含んでいました）。これらの要因は、小児患者に対してST を使用した ACL 再建後の高い再断裂率に関連している可能性があります。また、成長とともにⅠ型コラーゲンの割合が急速に増加していることが明らかとなり、移植腱にSTを使用した場合の年齢的なリスク領域を明らかにするために研究を進めております。

腱・靭帯損傷治癒過程促進

　膝前十字靭帯損傷に代表される関節内の靭帯損傷は自然治癒することが少なく、靭帯縫合術の成績も不良であるため、靭帯再建術が行われています。靭帯再建術は、自身のハムストリング腱や膝蓋腱を用いるため、手術が成功したとしても、選手が受傷前の状態に戻るのは難しいといわれています。
　そこで当科では、靭帯や腱がもつ再生能力に注目し、靭帯再建術に替わる治療方法の開発を目指して東海大学工学部、福井医療大学と共同研究を行っています。

1. アキレス腱由来＂gel＂の再生能力の評価
　マウス、ウサギのアキレス損傷部の断端より分泌された＂gel＂を回収し、その＂gel＂に張力をかけることで新たな腱へと再生する性質を発見しました。より再生能力の高い＂gel＂の性質について研究を重ねています。

2. アキレス腱由来＂gel＂の治癒促進能力の評価
　再生能力の高い＂gel＂を靭帯損傷部に添加し、正常の靭帯損傷の治癒過程と比較することで＂gel＂がもつ治癒促進効果の評価を行っています。

半月板再生

　以前は外傷による半月板断裂や加齢に伴う変性断裂に対しては、関節鏡を用いた部分切除～亜全摘術が行われていました。しかし、切除した半月板は元の形状に戻らず、長期間の経過で関節軟骨の損傷や、関節が変形する変形性関節症が生じる可能性高いことが問題となっています。そのため、近年では半月板機能の重要性が再確認されており、断裂半月板は可能な限り縫合し、温存する考えが広がってきています。しかし、断裂の形状によっては縫合が不可能な場合や、欠損が大きい場合はやむを得ず部分切除になるケースが少なくありません。また、半月板縫合後の再断裂など問題点が多いのが現状です。
　そこで当科では、ウサギの膝半月板を用いて欠損した半月板を再生する研究をしています。複数系統の細胞に分化できる能力（多分化能）と多分化能を維持できる能力（自己複製能）を持ち、なおかつ周囲組織を刺激し自己治癒能力を促進する効果も期待できる脂肪由来幹細胞を用いた半月板再生の研究をしています。

1. 脂肪由来幹細胞シートを用いた半月板治療の検討
　ウサギの膝半月板に欠損部をつくり、欠損部に脂肪由来幹細胞をシート状に作成した三次元構造体を移植して半月板の再生促進を試みています。まだ、動物実験のレベルですが、脂肪由来幹細胞の移植で半月板の再生が促進されることが証明され、国内外の学会で発表しています。

2. 脂肪由来幹細胞シートを用いて再生された半月板様組織の力学的特性の評価
　ウサギの再生された半月板に関しては、正常な半月板とは見た目は異なっているものの、半月板と同様な機能を有していることが期待できます。そのため、今後の臨床応用に向けて、再生された半月板に対する力学試験を行なっています。

ポジトロン断層撮影法(PET)を用いた変形性膝関節症患者の歩行による骨格筋代謝の評価

　臨床において、変形性膝関節症（膝OA）には症状がない無症候性膝OAも含まれており、これは骨の変形が必ずしも膝関節症状の要因ではないことを示唆しております。当科では、膝関節症状の有無には歩行時の骨格筋活動の違いが関与しているのではないかと考えました。
　骨格筋活動の評価には、筋電図検査を用いる方法が有用ではありますが、電極を装着した表層筋の評価は可能であるものの、深層筋を含めた（下肢全体の）骨格筋活動の評価は困難といった制限があります。そこで我々は FDG－PETを用いた評価方法に注目しました。これは、FDG（18F-fluorodeoxyglucose）を体内に投与して、骨格筋のFDGの集積を観察することで筋活動量の評価が可能な方法です。
　これまでの研究で、膝OA患者を膝関節症状の有無で2群に分類して歩行時の骨格筋活動の違いを FDG-PET を用いて評価したところ、特に中殿筋、小殿筋、股関節外旋筋について、症候群が無症候群よりも骨格筋活動が低い傾向がみられました。このことを踏まえて症候性膝OA患者では、股関節安定性が低下した状態で歩行することが　膝関節症状に影響している可能性があると考えられました。

臨床研究

スポーツ整形外科研究室では様々な臨床研究を行っています。その一部を紹介させていただきます。

オリジナル前十字靱帯（ACL）再建術の手術術式と成績に関する研究

解剖学的なACL再建術を目指してオリジナルダイレーターを開発し、実際の手術で使用しています。最近欧米で注目されている大腿四頭筋腱を使用したACL再建術にも取り組み、臨床成績などを検証しています。また、ACL再建術後の疼痛管理について調査を行い、なるべく疼痛が少なく、かつ筋力低下をおこさない方法を模索しています。さらに、当院放射線科との共同研究で、UTE T2*マッピングという新しいMRI撮影法を用いて半腱様筋腱と大腿四頭筋腱の靱帯化の違いを数値化する研究も行い、多くの成果を報告しています。

前十字靱帯再建術に使用する移植腱の線維径やコラーゲンの組成に関する研究

当科では、前十字靱帯再建術には自家半腱様筋腱を第一選択として使用しています。しかしながら若年者において半腱様筋腱を使用した前十字靱帯再建術は、再断裂率が高いことが世界的にも知られており、前十字靱帯再建術における課題のひとつでした。そこで当研究室では、半腱様筋腱や大腿四頭筋腱の線維径を年代別に計測し、若年者の半腱様筋腱は大腿四頭筋腱と比較して有意に細いこと、線維径は成長に従い大きくなることを発見しました。また最近では、免疫染色を利用して移植腱のコラーゲンの組成割合を計測しています。その結果、半腱様筋腱のコラーゲンの組成割合は成長とともに変化している可能性を見出しました。大腿四頭筋腱や膝蓋腱も同様の手法でコラーゲンの組成割合を計測しており、大きな成果を報告できると考えています。

早期変形性膝関節症に対する超音波検査の有用性に関する研究

近年注目されている、早期変形性膝関節症（早期膝OA）の病態解明と新しい治療開発を目的とした、金沢大学附属病院と関連施設との多施設共同研究です。早期膝OAは単純X線像で明らかな異常が見られないものの、軟骨の変性や半月板の損傷など、関節構造物に何らかの構造的変化をきたしている状態と報告されています。金沢大学スポーツ整形外科研究室では以前より簡便で低侵襲な超音波装置を使用して、患者さんの痛みに対して様々なアプローチで治療を行ってきました。早期膝OAに対しては、単純X線像では描出できない関節水腫や軟骨棘、半月板の逸脱などを描出することが可能です。これらの超音波所見とMRI所見や臨床スコアなどとの関連性を調査し、金沢大学から早期膝OAの診断や治療に有用な超音波所見/スコアを発信することを目標としております。
